- The participant can ask to see records and receive a copy.
- All information obtained will be kept confidential.
- This information is used to set up the Service Agreement with Healio Care Support Services.
- The Service Agreement will be signed off by both the Participant/Advocate, and Healio Care Support Services.
- A signed Service Agreement is required to start ongoing services with Healio Support Services.
- Information will be archived safely while the participant is receiving our services.
